Following the entry form should be a 200-word overview of the nomination. Following the brief overview should be a summary of not more than three typewritten pages that describe the project’s, program’s or individual’s contribution to urban and community forestry as outlined in the Award Categories.

This summary and overview can be saved in a Word document, Text document or PDF. File extensions accepted online include: .doc, .docx, .txt, .pdf and .wpd. Example: NameOfProjectSummary.docx

STEP THREE

If you do NOT have additional support materials, skip this step.

Support documentation such as photographs, press clippings, printed pieces, and letters of commendation are strongly encouraged, but shall be limited to 8 additional items. These files may have the file extensions of .pdf, .jpg, .doc, .docx, .txt., .wpd, sitx or .zip. Example: NameOfProjectAdditional1.jpg, NameOfProjectAdditional2.pdf, etc.

We recommend sending separate files. If you have the capability of compressing or zipping a folder containing the files (.sitx or .zip), this is also acceptable. Do NOT send a folder that has NOT been compressed. Good Example: NameOfProjectAdditional.zip.

Any file extensions listed above will be accepted. Please make sure the file(s) you are uploading in this step do NOT have the exact same name as a file previously uploaded. The file will be overwritten and cannot be recovered.
